Alliance process in a successful case of online psychodynamic psychotherapy for anxiety

dc.descriptionThe therapeutic alliance (TA) is one of the main predictors of good psychotherapeutic outcomes. Currently, intensive studies of the TA process in online psychotherapy are scarce. This study aimed to describe the TA process throughout the online treatment, considering indicators present in the psychotherapeutic process in general and the perception of the dyad regarding its level throughout the sessions (macroprocess). Studies involving the multilevel analysis of TA contribute to a better understanding and establishment of this in treatments. This was a naturalistic design through a systematic case study. A psychoanalytic therapist and a young adult patient with complaints of anxiety symptoms participated. The treatment consisted of 24 online sessions, videotaped, and transcribed and its outcome was considered successful. Data were collected using the Psychotherapy Process Q-Set, Working Alliance Inventory and the Rupture Resolution Rating System and analyzed using descriptive data. The results indicated a general collaborative process and high values on the part of the patient and the therapist in the domains of the TA (tasks, goals, and bond). In the microprocess, there were more avoidance disruptions and greater global impact of resolutions than there were disruptions. These findings were discussed from a psychodynamic perspective and constitute idiosyncrasies of care with this public. Finally, the limitations and suggestions for further studies regarding the alliance process in online psychotherapy are discussed.en-US
dc.descriptionAlianza terapéutica (AT) es uno de los principales predictores de buenos resultados psicoterapéuticos. Actualmente, los estudios intensivos del proceso de AT en psicoterapia en línea son escasos. Los estudios que involucran el análisis multinivel de la AT contribuyen a una mejor comprensión y establecimiento de esta en los tratamientos. Este estudio tuvo como objetivo describir el proceso de AT a lo largo del tratamiento en línea, considerando indicadores presentes en el proceso psicoterapéutico en general y la percepción de la díada de su nivel a lo largo de las sesiones (macroprocesos), así como su variación en las sesiones (microproceso). Es un diseño naturalista a través de un estudio de caso sistemático. Participaron una terapeuta de orientación psicodinámica y una paciente, adulta joven, con queja de síntomas de ansiedad. El tratamiento consistió en 24 sesiones en línea, grabadas en video y transcritas, y su resultado se consideró exitoso. Los datos fueron recolectados usando el Psicotherapy Process Q-Set, Working Alliance Inventory y el Rupture Resolution Rating System y analizados usando datos descriptivos. Los resultados indicaron un proceso colaborativo y altos valores por parte del paciente y el terapeuta en los dominios de AT (tareas, objetivos y vínculo). En el microproceso, hubo más interrupciones por evasión y un mayor impacto general de las resoluciones que de las interrupciones. Estos hallazgos fueron discutidos desde una perspectiva psicodinámica y configuran idiosincrasias del servicio brindado a esta audiencia. Finalmente, se señalan limitaciones y sugerencias para estudios adicionales sobre el proceso de alianza en psicoterapia en línea.es-ES
dc.descriptionA aliança terapêutica (AT) é um dos principais preditores de bons desfechos psicoterapêuticos. Atualmente, estudos intensivos do processo de AT na psicoterapia on-line são escassos. Este estudo objetivou descrever o processo da AT ao longo do tratamento on-line, tendo em vista indicadores presentes no processo psicoterápico em geral e a percepção da díade sobre o seu nível ao longo das sessões (macroprocessos), bem como a sua variação nas sessões (microprocesso). Estudos que envolvem a análise multinível da AT contribuem para melhor compreensão e estabelecimento desta nos tratamentos. Trata-se de um desenho naturalístico por meio do estudo de caso sistemático. Participaram uma terapeuta de orientação psicanalítica e uma paciente, jovem adulta, com queixa para sintomas de ansiedade. O tratamento foi composto de 24 sessões on-line, videogravadas e transcritas e o seu desfecho considerado bem-sucedido. Os dados foram coletados por meio do Psychotherapy Process Q-Set, Working Alliance Inventory e do Rupture Resolution Rating System e analisados utilizando dados descritivos. Os resultados indicaram processo colaborativo e altos valores por parte do paciente e da terapeuta nos domínios da AT (tarefas, objetivos e vínculo). No microprocesso, houve mais rupturas de evitação, e impacto global maior das resoluções do que das rupturas na AT. Tais achados foram discutidos sob a perspectiva psicodinâmica e configuram idiossincrasias do atendimento com esse público. Por fim, são indicadas as limitações e sugestões para novos estudos sobre o processo da aliança na psicoterapia on-line.pt-BR
