To convert the measurements obtained from the corneal surface into dioptric value, the B&L keratometer uses the general lens formula (n’-n/R) and assumes an n’ of 1,3375 (compared to the actual corneal refractive index of n’=1,376). Absenteeism, frequent phenomenon in business, is derived from various causes and has a negative impact on the economy and the development of the workers’ normal activities; it results in an increase in labor costs and workload. <em><strong>Objective:</strong></em> To determine the motivations of health professionals at the Hospital Vista Hermosa level I, health institution of the State, to be absent from their scheduled shifts. Materials and <em><strong>Methods:</strong></em> This study, aiming to reach technical importance, requires the qualitative and quantitative analyses of the variables involved in employee satisfaction at the immediate medical attention center (CAMI) Vista Hermosa. <em><strong>Results:</strong></em> The level of permanence of the working relationship with the company is high: 42% of time worked between two and ten years, followed by 33% of time worked for over ten years. A high degree of satisfaction was found (42%), followed by a 25% of very satisfied employees. This finding relates to the motivations and guarantees provided by the hospital to its employees. Conclusions: This study showed that the major causes of absenteeism at the CAMI Vista Hermosa were sick leave, delay or absence due to additional work and domestic calamity; which is directly associated with the fact that the company belongs to the health sector, where there is a high workload and a culture of having simultaneously one or more jobs. - Access barriers of pregnant Wayuu indigenous women to their prenatal controls before twelve weeks of gestation in the Hospital Nuestra Señora del Perpetuo Socorro in Uribia, La Guajira

This article seeks to identify the main barriers that prevent pregnant Wayuu indigenous women linked to the ESE of the municipality of Uribia, Hospital Nuestra Señora del Perpetuo Socorro, from access to their prenatal controls before the first twelve weeks of gestation, as regulated by legislation. This is a descriptive study with a transversal qualitative approach, which aims to contribute to a better management of difficulties faced by these pregnant indigenous women. To this effect, it is important to identify the main aspects that prevent access to their prenatal controls, as well as possible risk factors. In addition, the study allows recognizing the quality of services provided to these communities, which helps to identify and evidence aspects for their possible solutions. - Access barriers to visual health

A number of factors and conditions hinder and restrict access to the health care system and its different services these barriers to access put at risk the health of people by affecting adequate processes. <em><strong>Objective:</strong></em> To carry out a literature review on barriers to access to the health care system and visual health services in Colombia and around the world. <em><strong>Methodology</strong><em> A literature review was carried out based on a search of the Medline, ScienceDirect, and Pubmed databases, as well as indexed public health journals and the websites of the Local Health Authority, the World Health Organization, the Pan American Health Organization, the UNESCO, and the Brien Holden Vision Institute. <em><strong>Results</strong><em> The main barriers related to demand, both in general services and in visual health, are the lack of perception on the need for service and lack of economic resources at the offer level, the existing policies constitute a real obstacle. <em><strong>Conclusions</strong><em> Awareness-raising in the population, together with the implementation of health policies that grant equal access to health care services, are fundamental to prevent people from being affected, to a large extent, by barriers related to demand or offer, regardless of their location or level of income. - Accommodation in patients from five to twelve years with mild to moderate refractive amblyopia: a pilot test

<em><strong>Objective:</strong></em> determine the accommodative state in refractive amblyopia patients between five and twelve years old. <em><strong>Methods:</strong></em> A sample of fifteen refractive amblyopic children was included, and a control group of fifteen children between five and twelve years of age. The control group patients were required to have a maximum VA of 0.32 LogMar without correction, both at distance and near vision, and an uncorrected refractive error. It took into account individuals with mild and moderate refractive amblyopia of both anisometropic and isometropic types, with a minimum VA of 0.4 and a maximum VA of 0.8 LogMar with appropriate optical correction. Subjects suffering from organic ocular alterations, cognitive problems, diagnosis of strabismus, strabismus surgery or trauma were excluded, as well as patients who had attended or at the time were attending vision therapy. The accommodative state was evaluated in each patient, measuring the Lag of accommodation, the flexibility and the amplitude of accommodation (AA) with three subjective (PU, MPD and ML) and one objective technique (MODAA, for its initials in Spanish). <em><strong>Results:</strong></em> AA measured with the four methods was statistically significantly lower in amblyopic patients (p < 0.05), being the difference more pronounced with the use of ML (4.72 D). However, with the objective MODAA technique, which has shown high reliability, the differences were not clinically significant (0.47). Conclusions: The accommodative state of refractive amblyopia patients between five and twelve years is lower when subjective methods are employed. Accommodative spasm is a multifactorial entity in which there is a hyperfunction of the ciliary muscle that induces myopia. It can develop after refractive surgery, generating reduced expectations and bothersome visual symptoms for the patient. In this article we report a case of accommodation spasm in the non-operated eye following Lasik, with refraction without cycloplegia OD + 0.25 OI -3.00 Sph and with cycloplegia OD -0.25 OI +0.25 Sph. The visual and refractive response was adequate to the treatment of relaxation and accommodation flexibility with positive lens and flippers. Therefore, the aim of this paper was to review postsurgical vision therapy and the etiology underlying this unusual condition. - Accommodative state in amblyopic and non-amblyopic patients from 5 to 12 years of age

<em><strong>Objective:</strong></em> To determine difference in the accommodative state between refractive amblyopic and non-amblyopic patients from 5 to 12 years of age. <em><strong>Methods:</strong></em> The sample included 142 children (46 with refractive amblyopia and 96 non-amblyopic) between 5 and 12 years of age the accommodative state was evaluated in each one of them by determining the accommodative lag with Nott retinoscopy, accommodative flexibility with flippers, and the amplitude of accommodation with a subjective (MPD) and an objective technique (MODAA). <em><strong>Results:</strong></em> A statistically significant difference was found between amblyopic and non-amblyopic subjects in the lag of accommodation (0.22, p < 0.001), accommodative flexibility (1.5 cycles, <em>p</em> < 0.001), and the amplitude of accommodation 1.6 D (<em>p</em> < 0.001), using the MPD technique. With MODAA, there was no statistically significant difference between amblyopic and non-amblyopic children for the dominant eye (0.02 D, <em>p</em> = 0.893), while for the non-dominant eye the difference was 0.38 D. <em><strong>Conclusions:</strong></em> Using objective methods to evaluate these three functions of the accommodative state in patients with mild to moderate refractive amblyopia, the values obtained were within the normal range. However, when compared with non-amblyopic patients, a statistically significant difference was detected, which does not necessarily lead to the diagnosis of amblyopic patients with inadequate accommodation. The most altered accommodative function in the non-dominant eyes of patients with refractive amblyopia was accommodative flexibility. At present time there has been an increase of patients making near vision activities with a great near vision demand in reading and computer activities. The clinical practice experience shows frequently that near working for long time in computer generates different symptoms that decrease visual comfort (Lovasik & Kergoat, 1988; Hayes, et ál., 2007). The most frequent associated symptoms related to computer work have been migraine, fatigue, astenopía, jump of lines, intermittent double vision, accommodation problems during the work and after work when patient tries to focus in far vision (Sterner, 2006; Iribarren, 2002; Sheedy & Parsons, 1990); according to the studies, they can be other related causes to a decrease in accommodative response tests: amplitude, accommodative response, flexibility, near point and relative accommodations (Bar et ál., 2000). It also been found that abnormal vergences states that can be associated to accommodative anomalies inasmuch as they maintain one narrow relation, but that can be surpassed with an integral treatment with certain procedures that in the research have shown to be efficient. - Acetato de leuprolida, agonista de la GnRH, retrasa la neurodegeneración en un modelo experimental de glaucoma

Glaucoma is the main cause of irreversible blindness worldwide. In short, it is a multifactorial progressive optic neuropathy that correlates with retinal ganglion cell death, optic nerve head disturbances, and visual field disorders. Leuprolide acetate have recently been reported to have neurotrophic properties, the aim of this work was to determine whether it´s systemic administration holds up the neurodegenerative process in an experimental glaucoma model. Wistar rats divided into three groups were included: 1) a control group, 2) a hyaluronic acid-induced glaucoma group, and 3) a hyaluronic acid-induced glaucoma group treated with intramuscular leuprolide acetate. The eye electrical responses to light were recorded by simultaneous full-field electroretinography, and the eyes were processed for histological study. The results showed an improvement in the electrical activity, a recovery of fibers from the optic nerve as well as a reduction of the reactive astrogliosis in the leuprolide acetate treated group. In short, leuprolide acetate is a new potential alternative treatment in glaucoma, as it holds up the neurodegenerative process. - Acute angle-closure glaucoma: emergency management by the optometrist

There is a clinical presentation of closed-angle or acute glaucoma with pain symptoms and rapid course, which has great potential for detection and timely treatment. This condition is caused by excessive influx between the iris and the lens that prevents the flow of aqueous humor from the posterior to the anterior chamber with progressive relative pupillary block, occluding the filtering portion of the trabecular meshwork and causing elevated intraocular pressure (IOP). The urgency is manifested by strong eye pain and decreased visual acuity, which can be adequately managed by the optometrist, in order to make a timely referral to ophthalmology, as the consequences of the progression of the disease, or an incorrect diagnosis, are irreversible in terms of loss of visual acuity and visual field, which considerably affects the quality of life of individuals. <em><strong>Objective:</strong></em> To present clinical parameters in order to identify, diagnose and manage the urgency of angle closure with a timely referral for eye treatment. The cornea provides about 70% of the eye’s refractive power this is where most of the refractive surgeries that exist today, such as LASIK, are practiced. For this reason, it is essential to perform all the pre-surgical and postsurgical exams available, since iatrogenic ectasias can be prevented. Likewise, the biomechanical aspects of the cornea after laser surgery, such as the percentage of altered tissue and residual stromal thickness, provide extremely important data to determine the future behavior of the cornea after procedure. In this case report, the patient presented with penetrating keratoplasty caused by unilateral post-LASIK ectasia. For this reason, he was referred to optometry for visual assessment and rehabilitation, where a complete examination was carried out and the existing alternatives for the correction of the case were analyzed in depth. Para el tratamiento del queratocono, la queratoplastia penetrante es una de las alternativas más usadas en pacientes que presentan un alto grado de irregularidad y de adelgazamiento corneal. Consiste en el remplazo del tejido corneal enfermo que puede tener un diámetro variable, con un espesor total correspondiente al receptor, utilizando una sutura doble o continua o con puntos sueltos.La finalidad de este tipo de intervención es reemplazar el tejido opaco por uno transparente que ópticamente permita una refracción más clara y uniforme .El caso que se presenta a continuación muestra las consideraciones y las complicaciones que se pueden presentar luego del manejo con queratoplastia penetrante, su posterior terapia farmacológica con corticoides de potencia alta y penetración media, lo que ocasionó un glaucoma esteroideo relacionado con la disminución del flujo trabecular, debido a la interacción farmacológica de los corticoides y las prostaglandinas, que a su vez tienen un papel estructural importante en el trabéculo. Estos factores llevaron a la implantación de una válvula de Ahmed, dispositivo artificial que mejora el flujo del humor acuoso hacia el espacio conjuntival a través de un tubo de silicón que se encuentra unido a un plato de polipropileno, y con ello evitar un daño posterior de las estructuras oculares internas, incluyendo la cabeza anatómica del nervio óptico. El objetivo de este caso es reportar la adaptación de lente de contacto de apoyo escleral en un caso de glaucoma esteroideo y queratocono, tratado con válvula de Ahmed y queratoplastia penetrante bilateral. - Adherence to Pharmacotherapy in Hypertensive Outpatients Attending the Hypertension Control Program in the Jorge Cristo Sahium Hospital, Norte de Santander (Colombia)

<em><strong>Introduction</strong><em> Hypertension is a chronic disease that impairs vital organs and alters the quality of life of patients, who must remain in drug treatment with hypotensive and vasodilator medicine to control their blood pressure. <em><strong>Objective</strong><em> To determine the degree of adherence of chronic hypertensive patients to ongoing drug therapy, as well as the relationship between their habits and attitudes with this chronic disease. <em><strong>Methodology</strong><em> Through a quantitative, descriptive and cross-sectional study based on the Morisky-Green test, a multivariate analysis of outpatients attending the hypertension control program in the Jorge Cristo Sahium Hospital, Villa del Rosario, Norte de Santander, Colombia was performed. <em><strong>Results</strong><em> The total amount of people included in the program is of 563 patients, among which a representative sample of 154 was chosen 87.34 % were women and 12.66% were men the average age was of 66.295 ± 11.393 years, and 62 (39.74%) patients completed the treatment. <em><strong>Conclusions</strong><em> The most commonly used drugs for high blood pressure were those from the antagonist pharmacological group of angiotensin II receptors (ARA II) and the angiotensin-convertingenzyme inhibitors (ACE inhibitors), together with diuretics and antiplatelet agents. Patients often discontinue their medications for certain periods of time, which causes their nonadherence to pharmacotherapy. - Administrative career: a work opportunity for optometrist in the public sector

In Colombia, optometrists may work in the public sector. Law 9009 of 2004 states «norms to regulate the public employment, the administrative career, the public management and other dispositions»; and it is implemented in order to have access to the citizen administration with a better personal and professional quality, satisfying then the general interests, the human resources professionalism, the responsibility of the public employee, and an increase in the efficacy levels, in the public function. Public employment, which is part of the public function, must be subjected to selection processes under the principles of merit, free concurrence, and equality in the entry, publicity, transparency, and specialization of the technical entities in charge of the selection processes, guaranty of impartiality in the entities in charge of the processes, reliability and validity of the selection mechanisms. The administrative career, as a technical system of personnel administration, wants to guarantee the efficiency in the public administration, and the stability and equality in opportunities for the access and promotion in the public position. <p>The purpose of this case report was to characterize adult foveomacular vitelliform dystrophy (AOFVD), and to establish the main differential diagnoses of this disorder. Adult vitelliform dystrophy was first defined by Gass in 1974 to describe a clinical entity of autosomal dominant inheritance and adult onset. It is characterized by the bilateral presence of subfoveal rounded yellowish lesions. The classic angiographic image is a ring-shaped hyperfluorescence surrounding a central hypofluorescent area. It presents clinical and electrophysiological characteristics that allow establishing the differential diagnosis with other pathologies, which is important, as sometimes it can be misdiagnosed as age-related macular degeneration, among others. Also, it corresponds to a disease that leads to low vision, so the diagnosis is important for timely treatment.</p> - Advances in Building an Information System in Health in Colombia

<em><strong>Introduction</strong><em> Information systems seek to facilitate the logistics of health care, allow health institutions to function effectively, help caregivers to act effectively, improve access to administrative, clinical and epidemiological data, both individual and collective, and simplify access to biomedical references. <em><strong>Objective</strong><em> To identify progress in building an information system at the national level. <em><strong>Methodology</strong><em> An exploratory, descriptive and observational study was performed in which qualitative methods are used to collect existing data for the identification and description of information systems in Colombia. Initially, a review was made of databases such as Lilacs, Google Scholar and SciELO over the past twenty years in the following countries: Venezuela, Cuba, Brazil, Colombia, Peru, Spain, Costa Rica and Mexico. <em><strong>Recommendations</strong><em> For the implementation and commissioning of a functional information system in the country, it is necessary to formulate general policies to facilitate national strategies aimed at generating projects, infrastructure development and maximizing resources. <em><strong>Conclusion</strong><em> Technological advances can improve and master the information, which means mastering processes and decision-making. - Afecciones oculares, su relación con factores de riesgo ocupacional y uso de elementos de protección personal en una empresa metalmecánica en Bogotá

Esta investigación descriptiva buscaba conocer si existe alguna relación entre los factores de riesgo físicos, químicos, mecánicos y el uso de Elementos de Protección Personal Visual (EPPV) en las afecciones oculares de los trabajadores de una empresa metalmecánica. La muestra por conveniencia fue de 100 trabajadores (50 expuestos a los riesgos y 50 no expuestos). Se elaboró una encuesta para identificar la exposición extralaboral a los factores de riesgo y la comodidad del trabajador con los EPPV; se diligenció una historia clínica visual y se llevó un formato para evaluar la calidad óptica de los elementos de protección visual. Como resultado, se observó que al valorar la óptica de los equipos no se encontraron aberraciones, sin embargo frente al confort, el 40% de los trabajadores usuarios reportaron que su EPPV estaba rayado y un 14% ajustado. Entre los riesgos visuales están, 45% humos metálicos, 44% esquirlas metálicas, 6% ácidos y 5% soldadura. Las afecciones oculares encontradas en el grupo expuesto fueron 42% Hiperemia, 26% secreciones y 18% blefaritis. En el no expuesto 38% Hiperemia, 24% conjuntivitis y 17% pterigion. El grupo expuesto usuario de EPPV presentó secreciones 8% (p=0,006) y conjuntivitis 6% (p=0,49) quienes lo utilizan de manera permanente presentaban hiperemia en un 20% (p=0,16) y secreciones en un 14% (p=0,12) y quienes tenían su EPPV rayado, presentaron hiperemia en un 16% (p=0,20) y conjuntivitis en un 4% (p=0,22). Hubo disminución de 6,6% en la agudeza visual lejana y próxima quizás porque el EPPV en un 40% de los casos estaba rayado. - Against Social Blindness

Blindness, as a social problem, is evident from the lack of interest in other people’s welfare, the inability to social empathy as a condition for collective coexistence and the use of social prejudice that make others invisible in other words, social blindness exists when you lose the ability to recognize others as actors in the social construction of values. <p>This descriptive research wanted to know prevalence of allergic conjunctivitis in two medical institutions in Bogotá and three in Cundinamarca from March 2006 to February 2007. 813 patients constitute the sample of this study. They attended to the optometry and ophthalmology consultations in the Rafael Uribe Uribe Hospital and Medical Center San Camilo in Bogotá, and to El Salvador Hospital in Unabté, San Rafael Hospital in Girardot and Mario Gaitán Yanguas in Soacha, these three in Cundinamarca. The data collection method was through the optometric examination during the months of the study in each one of the above mentioned hospitals, after the exam standardization. As a result, it was observed that allergic conjunctivitis prevalence was 21.8% in the group of age between 0 – 17, 18% in the group of age between 18 -35 and 9.8% in the group of age between 36 – 53 and women presented the highest prevalence with 15.5%.</p> - Alteraciones del sistema antioxidante en retinopatía diabética

The diabetic with a deficient blood glucose control have an antioxidant activity total diminished, what causes defects in the protection against the free radicals of oxygen, specifically is altered the susceptibly al oxidative damage. The alteration of the glucose implies increase production of sorbitol and of glutation and may cause to damage in crystalline and retain, involving to a partial or total loss of the vision. Clinical trials diverse favor the hypothesis that the activity of the enzymes of the antioxidant system, are associates with microvascular damages in it retain, nevertheless, some authors have reported studies where there is not a statistically significant association, consequently, is important to present studies that permit to measure the antioxidant state total since this is a reflection of the free levels of radicals and of the ocular damage. With this article intends to carry out a revision on the role and the possible implication, to preventive level, that can have the association among alterations of the antioxidant system and the diabetic retinopathy; therefore would be able to determine, of indirect form, the start and the severity of the damage in retaining, which would permit to continue an early therapeutic conduct generating primary prevention.
